Reported event: an event regarding altr/ abnormal ion level and corrosion involving a restoration modular body was reported. The device failure mode was not confirmed.Method & results: -product evaluation and results: material analysis, visual, functional and dimensional inspections could not be performed as the device remained implanted.-clinician review: a review of the provided medical information by a clinical consultant indicated: this inquiry reports the failure of a revision total hip replacement after about 10 years due to elevated ion levels, corrosion and pseudotumor.I can confirm that this event took place since i was able to review the operation report and the stickers of the implants used.Regarding the possible root cause of this event, i cannot determine it with certainty.Causes of elevated ion levels, corrosion and pseudotumor and osteolysis are multifactorial. These include surgical technique factors, patient factors, and implant factors. In order to definitely confirm corrosion a metallurgical analysis should be carried out.-product history review: review of the device history records indicate devices were manufactured and accepted into final stock with no relevant reported discrepancies.-complaint history review: there have been no other similar events for the lot referenced. Conclusions: it was reported that the patient's hip was revised due to corrosion and adverse tissue reaction, osteolysis of the right total hip replacement and a large pseudotumor. The exact cause of the event could not be determined. No further investigation for this event is possible at this time.If devices and / or additional information become available to indicate further evaluation is warranted, this record will be reopened.
