Event Description
It was reported that a 1000ml eva (ethyl vinyl acetate) tpn (total parenteral nutrition) bag leaked from a hole.This was identified when storing the bag in the refrigerator.The overpouch appeared sealed and untampered; there were no signs of puncture noted.The bag contained standard preterm solution.There was no patient involvement.No additional information is available.

Additional information was added to h3, h4, h6 and h10.H10: the actual device was not available; however, a photograph of the sample was provided for evaluation.  visual inspection was performed to the photograph using the naked eye which revealed a leak from the upper back side of the bag.The photograph showed what appeared to be a puncture hole only in the back top of the bag approximately 1.0 inch below the hanger hole area.The reported condition was verified.By the nature of the sample, no additional tests were performed.  the cause of the condition could not be determined.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
