Description of event according to initial reporter: (b)(6) 2021 - physician had an extremely difficult ivc filter removal the other week.He said it was difficult to remove because he remember having problems deploying it when it was inserted.Patient outcome: according to the initial reporter, the patient did not experience any adverse effects due to this occurrence.The patient did not require any additional procedures due to this occurrence.

Additional information received 21oct2021: the hook did not let go after he pushed the release button.One in particular he placed and it was placed very tilted.As a result it took him 6 hours to retrieve the filter.

Manufacturer ref# (b)(6).Summary of investigational findings: the hook did not let go after pushing the release button and the filter was placed very tilted.The tilted filter was very difficult to retrieve, but finally retrieved after six hours without any reported harm to the patient.No product was returned and without the actual complaint device, based on the limited information provided it would be inappropriate to speculate at what may or may not have caused the reported difficulties in releasing the filter from the jugular introducer.The instructions for use supplied with the device specify that after stabilizing the filter introducer for filter placement, the protection sheath and the jugular introducer handle must be in contact with one another and at this point the filter is expanded, still connected to the filter introducer.And following, when the filter is in correct position and the red safety button is pushed to prepare filter release, the ifu warn that excessive tension during deployment may prevent the filter from releasing when the release mechanism is activated.There are adequate controls in place to ensure the device was manufactured to specifications.Cook medical will continue to monitor for similar events.This report is required by the fda under 21 cfr part 803.This report is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement made in it is intended to be an admission that any cook device is defective or malfunctioned; that a death or serious injury occurred; or that any cook device caused or contributed to; or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
