Additional Manufacturer Narrative
H.6.Investigation: our quality engineer inspected the sample and photograph submitted for evaluation.Bd received one photo and one used introducer in three portions: portions 1 and 2 were the handles that had broken apart and separated from the sheath, portion 3 was the sheath with a picc line threaded through it.Visual observation of the portions reveal that the unit had an incorrect peel which resulted in a jagged separation of the tabs and a break from the tubing.Your reported defect was confirmed.Further microscopic observation revealed that there was no skive line present on the length of the sheath indicating the defect is most likely related to the raw material.A device history record review showed no non-conformances associated with this issue during the production of this batch.
