Event or Problem Description
The patient was undergoing a medical procedure in the venous sinus using a benchmark 6f 071 delivery catheter (benchmark).During the procedure, the physician advanced the benchmark to the target vessel using a guidewire; however, he decided to replace the guidewire with a longer guidewire. While advancing the guidewire into the benchmark, the physician felt resistance at the distal end of the benchmark and therefore decided to retract the benchmark out. While retracting, the physician noticed several black dots on the images under fluoroscopy and realized that the tip of the benchmark was broken and unraveled. The physician then used a stent retriever to retrieve the tip of the benchmark successfully. The physician aborted the procedure at this point. There was no report of an adverse effect to the patient.

Additional Manufacturer Narrative
Evaluation of the returned benchmark could not confirm the distal shaft fracture and revealed that the distal tip was damaged and deformed.During functional testing, a demonstration guidewire was unable to be advanced through the returned benchmark due to the distal tip damage. If the benchmark is manipulated against resistance, damage to the catheter may occur. It is likely the retrieval attempt caused additional damage to the catheter tip.The marker coil was unwound on the distal tip of the catheter. This was likely the fracture reported.Penumbra catheters are inspected during in-process inspection and during quality inspection after manufacturing. The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.H3 other text : placeholder.
