Information was received from a healthcare provider via a manufacture representative regarding a patient receiving dilaudid (15 mg/ml at minimum rate) via an implanted infusion pump.It was reported a pump and catheter replacement were scheduled due to eos.Multiple attempts were made under fluoroscopy and intrathecal dye by the hcp to place the catheter but could never verify csf return, so attempts were abandoned. The new pump was implanted and set to minimum rate. The patient is being referred to a neurosurgeon for future placement of the catheter. There was no environmental/external/patient factors that may have led or contributed to the issue. The issue was not resolved.
