Event Description
It was reported that shaft break and balloon detachment occurred.A 2.00mm x15mm nc emerge balloon catheter was selected for use.However, upon removal of the balloon catheter from the hoop, the shaft was broken and the balloon fell off the shaft.The device was never used in the patient.No patient complications were reported.
