Patient allegedly received an implant on (b)(6) 2007 due to deep vein thrombosis (dvt), followed by an unsuccessful retrieval attempt on (b)(6) 2008.Patient is alleging vena cava perforation.The patient further alleges anxiety, depression, bipolar disorder, worry, fear, and physical limitations.Report from ct (computed tomography): "this dictation refers only to the ivc filter.Cook gunther tulip filter.Caval perforation: yes.2 o'clock 7.10 mm grade 2.Tilt: no.Migration: no.Pertinent negatives: none.".
