The following information is alleged: the patient received a celect inferior vena cava (ivc) filter on (b)(6) 2015.The patient underwent a computed tomography (ct) approximately two years and five months later, which revealed the filter had moved and several struts perforated through the ivc wall.Hospital and medical records have been requested, but not yet provided.

Patient allegedly received an implant on (b)(6) 2015 via the right internal jugular vein due to deep vein thrombosis (dvt) and pulmonary embolism (pe).Patient is alleging vena cava perforation.Report from ct (computed tomography): "inferior vena cava filter in place.The prongs extend beyond the inferior vena cava in the retro peritoneal fat.No associated retroperitoneal hemorrhage.".

"impressions: the filter is tilted posterior with its proximal cone against the ivc wall.Axial image 34.The anterior strut penetrates 10 mm through the ivc wall.It penetrates into the duodenum.Sagittal image 45.The left strut penetrates 12 mm through the ivc wall.It penetrates into the aorta.Coronal image 45.The posterior strut penetrates 8 mm through the ivc wall.Sagittal image 43.The right strut penetrates 12 mm through the ivc wall.Coronal image 42.Two arms each penetrate 8 mm through the ivc wall.".
