Event Description
|
Information was received from a healthcare provider via a company representative regarding a patient receiving gablofen 2000mcg/ml at 1592.6 mcg/day via an implantable pump.It was reported the pump was alarming and a motor stall occurred.Per the healthcare provider, the patient¿s pump ¿malfunctioned¿ and the patient¿s mother took the patient to the hospital.The patient presented to the er (emergency room) with pump alarming.The patient had 2 motor stalls with no mri.Per the logs a motor stall occurred (b)(6) 2021 at 1:54 am with a recovery at 3:25 am and a motor stall at 6:28 am.There were no environmental, external or patient factors that may have led or contributed to the issue.The pump was replaced due to the 2 motor stalls.The issue was resolved at the time of the report and it was noted that the healthcare provider would not have any further information regarding the event.

|
Additional information was received from a company representative (rep) indicated the pump was beeping but no alarm was reported into the logs.The patient was with physician and nurse interrogating the pump.No apparent causes were triggering the alarm.The patient was not showing any therapeutic issues.Alarms goes off every 10 minutes.It was noted the pump would be substituted with a new one on the date of this report.Additional information received indicated the pump had motor stall when logs were read as previously reported.
