Block h6: medical device problem code a0501 captures the reportable event of fiber tip detached.Block h10: investigation results: the returned flexiva id 1000 laser fiber was analyzed, and a visual evaluation noted that it was returned in one piece. The piece measured 277.4 cm from the end of the connector to the distal end of the fiber. The exposed glass tip was detached/separated from the fiber body and not received.Examination under magnification confirmed the pattern of the tip was consistent with a fracture.Functional analysis revealed that the light from the sma connector was bright and round, indicating no fracture within the fiber connector. No other problems with the device were noted. The reported event was confirmed. It is likely that procedural conditions and handling of the device during use contributed to the fiber tip damage. The ifu states, carefully remove the fiber from the package.Handle the fiber with care as damage may occur if struck or bent sharply.Inspect and treat the output tip with particular care as it represents the most delicate, easily damaged portion of assembly.Review and analysis of all available information indicated that the root cause is adverse event related to procedure.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.A labeling review was performed and, from the information available, this device was used per the instructions for use (ifu) / product label.
