Event Description
It was reported that an unknown patient underwent a paroxysmal atrial fibrillation (afib) ablation procedure with a carto 3 system.There was a map shift.It was reported during an afib there was a map shift without any error message, today, no harm on patient, used a new map to finish.No system error.The issue was seen during mapping.There was no patient movement.Map shift is mdr-reportable.

On (b)(6)-2021, the product investigation was completed.It was reported that an unknown patient underwent a paroxysmal atrial fibrillation (afib) ablation procedure with a carto 3 system.There was a map shift.It was reported during an afib there was a map shift without any error message, today, no harm on patient, used a new map to finish.Device evaluation details: the data related to the reported issue was provided.It was found that mapping was performed with alternating metal conditions, and it caused map shift.However, no warning was issued to the user as the metal level was below threshold.The field service engineer confirmed that the complaint was opened only for reporting issues and the system is operational.A manufacturing record evaluation was performed for the system # (b)(6), and no internal action related to the reported complaint condition were identified.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
