Additional Manufacturer Narrative
H10: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: one atlas pta dilatation catheter loaded onto an unknown guidewire has been received for the evaluation. On the visual evaluation, the device was noted bloody and no other visual anomalies were noted. On the functional evaluation, an attempt was made to remove the guidewire but the guidewire was stuck and inadvertently broken within the catheter. The broken segments of guidewire were able to be removed from the guidewire luer and distal end of the catheter. The guidewire lumen was flushed and an attempt was made to insert an in-house guide-wire however resistance was felt.Further, an x-ray was taken and a portion of the returned guide-wire was noted to be detached and stuck within the catheter. No further functional testing was performed. Therefore the investigation for the reported difficult to remove was confirmed, as the guidewire was returned loaded within the catheter. The investigation for the reported failure to advance and device-device incompatibility remains inconclusive, as the returned guidewire got detached and stuck within the inner guidewire lumen during the functional testing.A definitive root cause for the reported difficult to remove, failure to advance and device-device incompatibility could not be determined based upon the provided information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.H10: d4 (expiry date: 08/2023), g3.H11: h6 (method, result).H11: section a through f ¿ the information provide by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : see h10.
