Section d9 - device available for evaluation? yes.Section d9 - date returned to manufacturer: 15th december 2021.Device evaluation: visual inspection under magnification revealed viscoelastic residue inside of the cartridge as well as the lens module.Further inspection revealed a lens was received stuck inside of the cartridge, that the lens was also overridden by the plunger rod, and that the cartridge tip had cracked. The handpiece was disassembled and the assembly was inspected; presenting with no assembly issues. The complaint issue could be confirmed; however, this may be attributed to an excessive amount of ovd and excessive force applied to the lens, which is suggested by the amount of viscoelastic residue inside of the lens module as well as the stuck lens in the cartridge tip, and cannot be confirmed to be related to a manufacturing issue. Therefore, no product deficiency could be determined. Conclusion: as a result of the investigation, there is no indication of a product quality deficiency. All pertinent information available to johnson and johnson surgical vision, inc.Has been submitted.
