Event Description
|
It was reported that the procedure was to treat a heavily calcified distal right coronary artery (drca).The 2.5x15mm nc trek balloon dilatation catheter (bdc) was crossing the lesion; however, the bdc was pushed at an angle causing the hypotube to separate.Resistance with anatomy was noted during advancement of the bdc.The separated portion was outside of the anatomy; therefore, the separated piece was simply withdrawn from the anatomy.Another 2.5x15mm nc trek was used to successfully complete the procedure.There were no adverse patient effects and no clinically significant delay.No additional information was provided.

The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents and/or complaints from this lot.The investigation determined the reported complaints appear to be related to operational context.There is no indication of a product quality issue with respect to design, manufacture or labeling of the device.
