This report is being submitted as follow up no.1, to update section h3.And to provide the completed investigation results. One 8 fr angio-seal evolution device was received for product evaluation. The hemostasis sheath was returned mated with the carrier tube assembly along with the header bag.No other components were returned for evaluation.Upon visual analysis, there was bloodlike substance found on the device. There was a kink observed, on the hemostasis sheath shaft just below the hub.The tamper tube is completely released from the device, but still attached to the suture. The device sleeve was in rear lock position with the color bands exposed.A portion of the carrier tube shaft appeared to be cut and located over the suture proximal to the tamper tube. The device body is separated from the hemostasis sheath hub.And subjected to microscopic analysis. There was plastic deformation caused by applying tensile forces at the cut portion of the carrier tube shaft.Additionally, hemostat marks were seen on the carrier tube shaft distal end.Based on the evaluation performed. The complaint can be confirmed, for the damaged carrier tube housing. The tamper tube was also completely released from the device. The exact cause of the breakage seen on the carrier tube shaft is unknown. However, it appeared to be caused, during deployment of device. As there are hemostat marks on the tube. The exact cause of the event experienced by the user cannot be determined. The device was in a conforming state when released from terumo control. There is no indication, that any manufacturing issues may have led to this event.Currently, no action is recommended, since this risk evaluation is within the predetermined limits in the failure mode and effects analysis (fmea).
