Additional narrative: reporter is a synthes employee.Part #: 04.647.874; synthes lot #: 8735607; supplier lot #: n/a; release to warehouse date: november 26, 2013; manufactured by: werk mezzovico; no ncr's generated during production.Review of the device history record(s) showed that there were no issues during the manufacture of this product, and any sub-components, which would contribute to this complaint condition.Visual inspection: the 3.7 ti cerv spine screw slf-tpng/va 14mm (part #: 04.647.874, lot #: 8735607) was received at us customer quality (cq).Visual inspection of the complaint device showed that the hexlobe on the screw head was deformed. The outer major thread was also deformed. The visual inspection also revealed that the thin metal ring reported in the allegation did not come from the screw.Functional test: a functional assessment was not performed with the complaint device due to the post manufacturing damage of the complaint device. Can the complaint be replicated with the returned device? unable to perform dimensional inspection: no dimensional inspection was performed due to post-manufacturing damage.Document/specification review: current and manufactured revisions were reviewed. No design issues or discrepancies were identified.Complaint confirmed? yes.Investigation conclusion: this complaint is confirmed as the visual inspection of the screw revealed that the hexlobe and the outer major thread were both deformed. The damage observed on the complaint device appeared to be post manufacturing. It is likely this damage occurred during the surgery and therefore contributed to the screw not being able to assemble with mating devices. No root cause could definitively be determined for the reported complaint condition. No new, unique or different patient harms were identified as a result of this evaluation. There was no indication that a design or manufacturing issue contributed to the complaint. No design issues were observed during the document/specification review.Based on the investigation findings, it has been determined that no corrective and/or preventive action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post market safety surveillance activities.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
