Event Description
It was reported that unspecified bd¿ venflon pro had a safety mechanism failure.The following information was provided by the initial reporter: it was reported via post market survey that clinicians encountered extravasation (not related to high pressure injection of contrast media) (2) and cannula occlusion (1).Customer response: it was a dirty needle stick injury.No re-capping happened.The guard on the tip of the needle came off and caused the injury.
