Additional information: section d9.Device available for evaluation? yes returned to manufacturer on feb 18, 2022 section h3.Device evaluated by manufacturer? yes device evaluation: the product was returned to the manufacturing site for evaluation.Visual inspection under magnification revealed that the preloaded device was returned damaged around the lower body assembly. The complaint lens was also observed stuck in the cartridge of the device and overridden by the plunger rod which was also observed to be bent.Dried viscoelastic residue was also observed expelled from the cartridge tip.The handpiece was reassembled to confirm that there was no issues with the assembled parts. Therefore, the observed gap may be attributed to an excessive force applied to the lens during advancement. The complaint issue "dc-stuck in cartridge" was confirmed (dc-lens damage was not confirmed); however, based on the complaint investigation results the complaint issue could not be confirmed to be related to the manufacturing or design process. Therefore, there is no indication of a product deficiency or product malfunction. Conclusion: as a result of the investigation there is no indication of a product quality deficiency and the reported issue could not be verified. All pertinent information available to johnson & johnson surgical vision, inc has been submitted.
