Additional Manufacturer Narrative
A united states customer contacted a siemens customer care center (ccc) to report a falsely depressed carbon dioxide (co2) patient result that was obtained on a dimension vista 500 instrument.A siemens customer service engineer (cse) was dispatched to the customer's site.During this visit, the cse replaced the sample mixer and the controller area network (can) board on the reagent 2 pump panel.Siemens further investigated the issue and determined that the co2 reaction was flat after the sample was added and the result monitor demonstrated sample mixer issues. Therefore, siemens concluded that the malfunctioned sample mixer potentially contributed to the falsely low carbon dioxide result. After the service visit, the instrument is performing according to specifications. No further evaluation of this device is required.

Event or Problem Description
A discordant, falsely depressed carbon dioxide (co2) result was obtained on a patient sample on a dimension vista 500 instrument. The discordant result was reported to the physician(s), who questioned the result. The sample was repeated on an alternate dimension vista 500 instrument. The repeat result was higher than the discordant result. The repeat result was reported, as the correct result, to the physician(s).There are no known reports of patient intervention or adverse health consequences due to the discordant, falsely depressed co2 result.
