Block h6: medical device problem code a040506 captures the reportable event of internal desquamation.Block h10: investigation results the returned navigator hd device was analyzed, and a visual evaluation noted that the sheath and the dilator were bent.A container was also returned with the internal lining of the sheath inside of it.Microscopic examination was performed and it was found that the internal diameter of the sheath had desquamation, thereby confirming the reported event.A functional evaluation was performed by reinserting the dilator through the sheath and no problems were observed. No other problems with the device were noted. The reported event was confirmed. It is possible that factors encountered during the procedure, such as handling and manipulation of the device during use could have caused the kinked found on the sheath.Once the sheath is kinked, additional devices or tools could rub the inner walls of the sheath and can damage the inner lining of the sheath which could have contributed to the reported event. Therefore, the most probable root cause is adverse event related to procedure since the adverse event occurred during the procedure and the device had no influence on the event.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.
