Event Description
This is being filed to report the atrial septal defect requiring intervention.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 4+.An intra-aortic balloon pump was placed prior to the start of the procedure.The clip delivery system (cds) was advanced to the mitral valve however the leaflets could not be grasped therefore the cds was removed.A second cds was advanced and deployed, reducing mr to 2+.Once the steerable guide catheter (sgc) was removed a right to left shunt was observed therefore an amplatzer asd device was successfully placed.The balloon pump was then exchanged for an impella device for further post-op support, and it was then observed that the patient blood pressure and vitals dropped.Attempts were made to improve the patients condition however the patient expired.Per the physician, the patient death was not related to the mitraclip devices.No additional information was provided.
 
Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Based on available information, the cause of the reported perforation could not be determined.The reported perforation as listed in the mitraclip system instructions for use (ifu) is a known possible complication associated with mitraclip procedures.The reported unexpected medical intervention is the result of case-specific circumstances.There is no indication of a product quality issue with respect to manufacture, design or labeling.
