Event Description
It was reported that, during a treatment setup, when an opsite flexifix 5cmx10m ctn 1 was removed from the carton, it was confirmed that the carrier peeled off the edge a few dozen centimeters.Treatment was completed with a backup device, without any delay.Patient was not harmed as consequence of this problem.
 
Manufacturer Narrative
H3, h6: the batch record was reviewed and it could be confirmed that the products had progressed through a satisfactory release process which involved testing the product against the requirements of the finished product specification.There was also no issue identified during the manufacturing process that could have caused or contributed to the complaint problem.A complaint history review found no similar instances of the reported event.The device was used for treatment and was returned for analysis.The device was found to be as described in the complaint, thus confirming a relationship between the reported event and the device.It was reported that the dressing was removed from the carton and was damaged and missing from the carrier paper for around a metre of the roll.Probable root cause is a missed quality check during manufacture which should have rejected the product and removed it from the process.No further actions by smith and nephew are deemed necessary at this stage.However, we will continue to monitor for any adverse trends relating to this product range.Smith and nephew acknowledge customer concern and are continually investigating ways to develop and improve our products.
