Additional Manufacturer Narrative
H10: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: one atlas gold pta dilatation catheter was returned for evaluation. The balloon was noted to be detached from the entire catheter. Both marker bands were noted to be within the guide wire lumen. As regards the balloon, frayed fibers and unravelling were noted. All anomalies were noted upon microscopic observation. No functional testing was performed due to device condition. One photo was reviewed. The photo shows the detached balloon which appeared to be bloody. No evidence of balloon rupture could be found on the submitted photo.Based on the photo review, the reported balloon detachment can be confirmed, however the reported balloon rupture could not be confirmed from the findings.Six radiographic images were reviewed.Fluoroscopic images show an angioplasty balloon inflated likely near the region of the left brachiocephalic vein.Subsequent images show an inflated angioplasty balloon. However, there is also evidence of balloon rupture.Another image also shows an object in a similar location of the balloon inflation that resembles retained balloon material. Therefore, based on the image review, the reported balloon rupture can be confirmed, however based on the submitted images detachment could not be confirmed from the findings and therefore no conclusion can be made.In summary, the investigation has confirmed the reported balloon detachment, as the balloon was returned in a detached condition. The submitted photo also confirmed the balloon detachment. The investigation has also confirmed the reported balloon rupture, as the submitted radiographic images have confirmed the balloon rupture. The investigation has also confirmed the reported frayed material and unraveled fiber, as visual and microscopic observation have noted the fiber disturbance.A definitive root cause for the reported balloon rupture, balloon detachment and identified frayed material and unraveled fibers could not be determined based upon the provided information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.H10: d4 (expiry date: 03/2023), g3, h6 (device).H11: b5, h6 (patient, method, result, conclusion).H11:section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : see h10.
