Information was received from healthcare professional via manufacturer representative regarding a patient having lumbar laminectomy and posterior interbody, with minimally invasive screws due to collapsed l5/s1 disc, disc herniation at l4/5 <(>&<)> l5/s1.It was reported that when attempting to implant the elevate cage, whilst hammering the implant into the disc space, the surgeon noted that the cage was 'opening'.Surgeon removed it and when ensure that the cage was close used the relevant driver.Surgeon attempted it again, but the same occurred again. The peek part of the cage was 'catching' and not going into the disc space. It was reported that the small part of peek at the rear of the cage has broken off, hence why the surgeon was reporting that the cage was opening. We selected one size height smaller cage, and prepared it in the same way and it inserted with no issues. There were no patient symptoms/ complications as a result of the event.

H3: product analysis: (b)(4), lot# 0448674w.A visual review of the implant confirmed the ¿ears¿ of the -03-top component are broken off and not returned for analysis.Microscopic review of the returned item indicates that are witness marks on the back side of the implant where it connects to the inserter, indicating force was used in implantation. There do not appear to be any significant witness marks on the nose of the peek portion, this indicates that it would appear the spacer was fully closed. The fracture surface at the ears indicates material overload.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.
