Event Description
Medtronic received a report that the patient presented with a large vessel ischemic stroke 6 days after pipeline implantation.The patient died the next day. the cause of death was hemorrhagic conversion of large territory ischemia stroke. angiographic results post procedure had shown technical success.The patient was undergoing treatment for an ruptured, saccular aneurysm located in the right ica.The max diameter was 5mm, and the neck diameter was 3mm.The patient's vessel tortuosity was moderate.The landing zone was 3.2mm distal, and 3.5mm proximal. dual antiplatelet treatment was administered.The pru level was 55 on the day of the procedure, and 191 when the patient presented with ischemic stroke. the devices were prepared according to the instructions for use (ifu).

Additional information received reported that a balloon was used to completely open the device during the procedure.Deployment was successful, and the patient was discharged home the next day.
