Event or Problem Description
A customer reported a healthcare worker (hcw) received a "low temperature burn" or skin reaction when she opened a new package of sterrad® 100nx cassette. The hcw was not wearing gloves, and she made contact with hydrogen peroxide before noticing the h2o2 indicator strip changed color. She experienced tingling sensation on her fingers for both hands. She rinsed both hands with running water, and it was reported symptoms disappeared at a later time.No medical attention or treatment was received for the h2o2 skin reaction and it was reported she recovered. This event is being reported as a malfunction report subsequent to a serious injury.

A visual analysis of field sample pictures was performed. The pictures showed an opened sterrad® 100nx cassette with the indicator bar changed from white to a dark red, and the cells of the cassette appear to be empty.Retain samples were visually inspected, and no signs of leakage through color change of the indicator bar was observed.Additionally, the rfid label check was performed and showed the correct variable data (product code, lot# and expiry date).Since the manufacturing record evaluation did not identify any deviations or non-conformances, it is highly unlikely that such a defect occurred during production or packaging. The report of color change and leakage was confirmed through field sample pictures; however, return of a physical field sample would be necessary to perform further analysis.Asp complaint ref #: cmp-(b)(4).
