Event Description
Medtronic received information regarding a pipeline flex with shield that failed to open and then because stuck in the phenom microcatheter and a second pipeline flex with shield that also failed to open.The patient was undergoing a procedure to treat a ruptured saccular left internal carotid artery (ica) supra clinoid aneurysm.The aneurysm max diameter was 5.91mm and the neck diameter was 4.23mm.The distal landing zone was 3.6mm and the neck diameter was 4.75mm.Vessel tortuosity was reported to be normal.It was reported that it was planned to deploy a pipeline from the middle cerebral artery (mca) to the ica.The pipeline flex (ped2-500-35) was delivered and deployment started but the device did not open at the distal end.The device was resheathed and redeployed but still did not open distally so it was decided to removed the pipeline.However it deployed and became stuck in the phenom 27 microcatheter.Another pipeline flex (model: ped2-500-30) was then tried with a new phenom 27 microcatheter.However once delivered, the ped2-500-30 also failed to open at the distal end.The device was removed.A pipeline (ped2-500-25) was then used which deployed and was implanted successfully.There was no harm or injury to the patient.

Event Description
Additional information received reported the pushwire was not rotated or pulled back.The pipeline was not in a vessel bend when the failure to open occurred; it was in a straight segment of the middle cerebral artery (mca).Resheathing was done to try and open the pipeline without success; no other devices or maneuvers were tried to open the pipeline.

H3.Product analysis: equipment used: video inspection system (m-85519), ruler (m-83361), camera (panasonic lumix dmc-zs5), in-house 0.0265in mandrel.As found condition: the pipeline flex shield was returned inside of the phenom 27 catheter; within the inner pouch and outer carton; inside of a sealed bio-hazard bag and a shipping box.Visual inspection/damage location details: the distal and proximal dps restraints were found to be intact.The dps sleeves were found intact with no signs of damage.The distal hypotube appeared to be stretched with the ptfe shrink tubing still intact.The distal end of the braid was not opened due to damaged braid.The proximal end of the braid were found fully opened and frayed.Bends were found at 20.0cm to 28.5cm from the proximal end.No defects were found with the tip coil, distal marker, re-sheathing marker, re-sheathing pad or with the proximal bumper.The total and usable lengths of the catheter were measured to be within specifications.The catheter tip and marker were examined; no damages were found.The catheter body appeared to be accordioned at 8.0cm to 15.5cm from the distal tip.No flash or voids molded were observed in the hub.No other anomalies were observed.Testing/analysis: the pipeline flex shield could not be pushed forward or removed.For further examination, the catheter was cut to remove the pipeline flex shield delivery system.The catheter was flushed with water and found patent.The catheter was then tested by running an in-house 0.0265¿ mandrel through catheter tip and hub.The mandrel successfully passed through the catheter hub with no issues; however, resistance was observed at the damaged locations.Conclusion: based on the returned devices, the customer's complaint was confirmed as the pipeline flex shield was found stuck inside the phenom catheter.The distal end of the braid was not opened due to damaged braid.The damage to the braid on the ends of the pipeline flex shield is likely the results of the physician re-sheathing the device more than recommended two times.From the damages seen on the catheter (accordioning), pushwire (bending), pipeline flex shield braid (fraying) and hypotube (stretching);
