Additional Manufacturer Narrative
The oad was received at csi for analysis.A guide wire could not pass through the oad and detailed analysis revealed that the driveshaft was overloaded at the lap weld.Driveshaft overloading is an indication that the spinning driveshaft met sudden resistance bringing it to a stall condition, however this could not be confirmed and the root cause of the driveshaft damage remains undetermined. The device data log identified stall events which confirmed the stopped spinning event. At the conclusion of the device analysis investigation, the report that a dissection occurred could not be confirmed through analysis, and the stopped spinning event was confirmed through the device data log.In the opinion of the physician, the cause of the dissection was unknown. The device history record for this oad lot number has been reviewed. No issues or discrepancies were noted during this review that would have contributed to the reported event. The device met material, assembly, and quality control requirements.(b)(4).

Event or Problem Description
The stealth peripheral orbital atherectomy device (oad) was operated on low and medium speed for two treatment passes each. The oad stopped spinning abruptly when operated on high speed. The oad was removed, and a type d dissection was observed.Balloon angioplasty was performed to resolve the dissection.A second oad was used to complete treatment to the distal area of the lesion. The patient was discharged without additional complications.
