Additional Manufacturer Narrative
|
|
Visual inspection was performed and found the distal end of the driver to be deformed rotationally.Deformation location and direction indicates deformation most likely occurred in a previous case during screw insertion.Manufacturing records were reviewed for the corresponding lot and no relevant issues were identified.Device was manufactured in june 2018 and was over 3 years old at the time of the reported event. No similar complaints for the reported lot number were identified.Device stg was reviewed: once the screw hole is prepared and the appropriately sized screw is selected, screws can be inserted into the plate using the size 10 tapered driver or the size 10 threaded driver. To use the size 10 threaded driver: the size 10 threaded driver has a threaded distal tip to aid in the retention of the screws during insertion.Attach the proximal end of the threaded driver to the torque limiting handle.Insert the distal end of the threaded driver into the screw head and turn the knob clockwise to engage screw. The threaded driver must be used with the 12-in lbs torque limiting handle. The threaded driver will not work with the fast guide the most likely cause of the reported event was determined to be multi-factorial.Device age and previous excessive torque most likely both contributed to the event.Lack of use of the torque limiting handle may have potentially contributed to event.
