Investigation results: as of the date of this report the complaint product was not provided for investigation. Therefore, a thorough investigation is not possible.Due to the constant monitoring of the compliance with our quality standards, as matters stand, a production or material defect can most likely be excluded.Possible complications of which the manufacturer is currently aware include leakage. For risks, adverse effects, interactions, indications and contraindications, instruction for use has to be observed.Since the exact location of the leakage is unknown, an application-related error during application cannot be ruled out.In order to avoid incidents, the following steps must be observed: choose the suitable implant size to fit the application area.Cut neuro-patch into the required shape. The implant should be cut as closely as possible to the defect size to achieve tension-free embedding.Fix the neuro-patch using non-absorbable suture material (polyester, polypropylene). The use of atraumatic round body needles permits suturing without major damage to the implant. In addition, fibrin glue can be used to achieve a sealing effect.During the application of neuro-patch in combination with bone cement, chemical damage of the patch material, depending on the application situation, cannot be excluded.Batch history review: the device quality and manufacturing history records have been checked for the available lot number and were found to be according to our specifications valid at the time of production.Review of the complaint history revealed that no similar complaints have been filed against products from this batch number. The review of risk assessment revealed that the overall risk level (severity 4(5) x probability of occurrence 1(5)) according to din en iso 14971 is still acceptable. Conclusion and measures / preventive measures: based upon the investigation results a clear root cause conclusion cannot be drawn. There is no indication for a material, manufacturing or design-related failure. In the event that the complaint product will be provided for investigation in the future, an update of this report will be provided unsolicited.Based upon the investigations results a capa is not necessary.
