Block b3: the exact date of the event is unknown. The provided event date was chosen as a best estimate based on the date that the manufacturer became aware of the event.Block d4, h4: the complainant was unable to provide the suspect device lot number; therefore, the lot expiration and device manufacture dates are unknown.Block h6: medical device problem code a27 is being used to capture the reportable event of aborted/cancelled procedure.Block h10: the returned radial jaw 4 biopsy forceps was analyzed, and a visual evaluation noted that the device was cut off with the jaws inside the scope part. The jacket presents damage from a sharp object and the clevis arms are bent. The reported event was confirmed.Based on all available information, it is possible that due to the manipulation or the technique used by the physician in conjunction with the patient's anatomy at the time to take biopsies could have affected the device operation. An excess of force could have been applied bending the clevis arms, then at the time of trying to remove the device, the arms bent even more, resulting in the inability to pass backwards. The evidence of the scope part and the device cut off indicates complications to remove the device. Therefore, the most probable root cause is adverse event related to procedure.A manufacturing batch record review was unable to be performed as the lot number is unknown. However, a ship history review was performed to identify the most probable lots, and a device history record (dhr) review on the most probable lots did not identify any deviations within manufacturing/service processes that could have contributed to the event.
