Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.H3, h4, h6: a manufacturing record evaluation was performed for the finished device, product code: 177492060, lot: ti0974.It was electronically reviewed and no non-conformances / manufacturing irregularities were identified during the manufacturing process. The product was released on: september 30, 2020.Visual inspection: the device was received at us customer quality for investigation.A visual inspection noted damage to the distal ends of the two set screws. There was also damage to one of the threaded holes inside the body of the device.Scratches were noted on the device that were consistent with implant removal.Functional test: no mating device was returned so a definitive functional test cannot be performed.Dimensional inspection: a dimensional inspection was not performed due to the geometry of the complaint relevant areas of the device.Document/specification review: the current and manufactured documents were reviewed as part of the investigation. No design or manufacturing discrepancies were noted. Conclusion: the complaint is not confirmed for the received device as the mating rod was not returned and therefore the loose condition cannot be evaluated. The investigation found damage to the set screws and one of the set screw threaded holes, but without the mating rod it cannot be determined if this damage would prevent attachment of connector to the rod. There was no indication that a design or manufacturing issue contributed to the complaint.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post market safety surveillance activities.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
