It was reported that the device had activation issues that caused the blades not to spin properly.A jetstream xc 2.4mm catheter was selected for use in a superficial femoral artery (sfa) atherectomy as treatment for peripheral artery disease.The physician activated the device to treat the patient, and the device was working as expected until the second rex mode would not stay activated with the bottom depressed.The blades were up and the device was not spinning properly and audibly noticed by physicians.All three rotation modes were again attempted, but the rotation was not able to be activated.There were no visible defects or error messages indicated at the time that this issue occurred.This device was not used, however another jetstream device was used successfully to complete this procedure without sequela.The device was discarded and not expected to be returned for analysis.
