Additional Manufacturer Narrative
Manufacturer¿s ref.No: (b)(4).The purpose of this mdr submission is to report the investigational finding of the returned device.[conclusion]: the healthcare professional reported that during the procedure, the 2.50mm x 4.50cm galaxy g3 mini coil (glm925045 / 30383054) became kinked in the prowler select lpes microcatheter. There was no report of any patient adverse event or complication.Multiple attempts to obtain additional information related to the procedure and the reported device issue were unsuccessful. If additional information is received at a later date, this file will be updated accordingly.A photo of the complaint device was included in the complaint file. The product analysis lab reviewed the photo of the complaint device provided in the complaint file. The observation is documented below.[photo analysis]: based on the photo that was included in the complaint file, it can be determined that the device positioning unit (dpu) of the complaint device is in a protruded condition. However, no damages could be observed on the dpu.It cannot be determined based on the photo if the introducer is damaged. No other defects could be observed. The embolic coil component was not shown in the photo. The reported issue that the embolic coil became kinked in the prowler select lpes microcatheter cannot be observed based on the photo.A review of manufacturing documentation associated with this lot (30383054) presented no issues during the manufacturing or inspection processes related to the reported complaint. There were no nonconformances related to device manufacture or inspection. All product rejected during manufacturing was identified as scrap and properly accounted for.As part of the cerenovus quality process, all devices are manufactured, inspected, and released to approved specifications. The manufacturing record evaluation does not indicate that the reported issue in the complaint is related to the manufacturing process. The complaint device was returned for evaluation and analysis. The investigational finding is documented below.Investigation summary: a non-sterile 2.50mm x 4.50cm galaxy g3 mini coil was received contained in a pouch. The device underwent visual inspection. The core wire / device positioning unit (dpu) was observed protruding from the introducer; this observation is consistent with the photo analysis which noted the dpu in a protruded condition. It was also noted that the dpu is bent near the marker band.Dried blood residues were observed along the device.Microscopic inspection was performed. The embolic coil component is observed to be in good, normal condition. It was still attached to the dpu with no elongations, kinks, and without any non-concentric loops. The core wire and the hypotube noted with the dried blood residues indicated that the device was used in the patient; however, the reported issue in the complaint is that the 2.50mm x 4.50cm galaxy g3 mini coil became kinked in the prowler select lpes microcatheter. The bent was observed on the core wire / dpu, not on the embolic coil component. Therefore, the reported issue documented in the complaint could not be confirmed.Attempts were made to obtain additional information related to the procedure and the reported issue was not successful, therefore, the cause / contributing factors that may have resulted in the reported issue is not determined.Based on the manufacturing documentation review, there is no indication that the event is related to the device manufacturing process. As part of the post market surveillance program, information from this complaint is trended for statistical signals and corrective / preventive action may be triggered at a later time.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time. The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be submitted within 30 days of receipt.
