Since no device has been received for analysis, no product investigation can be performed, and the customer complaint cannot be confirmed.Manufacturing record evaluation (mre) cannot be conducted because no lot number was provided by the customer.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.(b)(4).
 
Event Description
It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a carto vizigo¿ 8.5f bi-directional guiding sheath ¿ small and a hemostatic valve separation issue occurred.It was reported that the vizigo¿ sheath "hub was busted" and "no blood or fluid could be pulled through the valve".The physician thought that the valve broke as she could not suction blood through it.The hemostasis valve (gasket) did not break into two or more separate pieces.The hemostatic valve/brim cap/hub did not become detached from the sheath.The sheath was being used on the patient.Air did not enter the patient¿s body.This issue did not require percutaneous or surgical removal.No blood return was observed.The vizigo¿ sheath was replaced and the issue was resolved and the procedure continued successfully.No adverse patient consequences were reported.
