It should be noted the gore® dryseal flex introducer sheath instructions for use (ifu) state ¿adequate vessel access is required to introduce the sheath into the vasculature.Careful evaluation of vessel size, anatomy, tortuosity, and disease state (including calcification, plaque, and thrombus) is required to ensure successful sheath introduction and subsequent withdrawal. If vessel is not adequate for access, major bleeding, vessel damage, or serious injury to the patient, including death, may result.¿.

On (b)(6) 2021, the patient underwent endovascular treatment of a thoracic aneurysm using gore® excluder® aaa endoprostheses.A 22fr gore® dryseal flex introducer sheath was also used as an accessory during the procedure. It was reported the 22fr sheath was inserted through the left common femoral artery for access.According to the report, the vessel was notably tortuous and narrow (measurement not provided), and resistance was felt while advancing the sheath.A dissection was reportedly identified at the insertion site, and surgical repair was required.A decision was made to access the right common femoral artery. The same 22fr sheath was inserted, and stent grafts were successfully advanced and implanted.Upon withdrawal of the 22fr sheath, a suspected dissection in the right common iliac artery was observed. An additional non-gore manufactured stent was implanted to treat the dissection on the right side. The patient tolerated the procedure.
