Event Description
(b)(6) hospital have reported that 2 x 940080007 are blunt and need to be replaced.This was reported on the 16th december, no delay in surgery.The hospital confirmed to me to day that they can not provide the serial numbers as the have disposed of them into the sharpes bin.
