Event Description
|
A user facility reported to olympus that during preparation for use, the endoeye flex deflectable videoscope screen became black and displayed no image.There was no harm or user injury reported due to the event.

Manufacturer Narrative
|
The device was returned to olympus for evaluation.The evaluation confirmed the image issue during angulation on the downward position.The device was received with a missing bending section cover.There were excessive broken strands (wires) observed on the bending section.A hairline crack was observed on the video connector.In addition, looseness between the insertion tube and switch unit on the body control unit was observed.The device has been serviced and returned to the user facility.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon "image goes off upon angulating on down" was presumed to have been caused by the following: i) stress was applied to the bending section of the subject device due to insertion into/withdrawal from the trocar.Ii) the bending segment incorporated into the bending section interfered the ccd cable.Iii) ccd cable incorporated into the bending section was kinked or broken.The suggested phenomenon "a-rubber is missing" (a-rubber meaning: bending section cover or distal sheath) was presumed to have been caused by stress applied to the bending section when inserting to/withdrawing from the trocar.Olympus will continue to monitor field performance for this device.
