Event or Problem Description
The event was reported via the (b)(6), a (b)(6) year-old male patient with a history of hypertension, coronary artery disease (cad), and active smoking underwent coil embolization of an unruptured anterior cerebral artery (aca) on (b)(6) 2021 and experienced a thromboembolic event during the procedure.Medication was administered as treatment. The patient was asymptomatic; the thromboembolic event was only observed angiographically. The patient was discharged home with self-care on (b)(6) 2021 with modified rankin scale (mrs) score of 0.Platelet reactivity testing was not performed. The dimensions of the aca bifurcation aneurysm were as follows: aneurysm height 3.7mm, dome 5mm, maximum aneurysm diameter 5.3mm, neck size 2.9mm, and dome-to-neck ratio of 1.7mm.The parent vessel diameter was 2.4mm.Coil embolization was performed with the implantation of two competitor (stryker) coils and a 4mm x 8cm galaxy g3 xsft (glx120408 / l13313) coil via an excelsior sl-10 microcatheter (stryker). There was microcatheter kickback (loss of access to aneurysm) experienced twice during the procedure.Heparin was administered during the procedure. In the opinion of the investigator, treatment of the target aneurysm was considered complete, and the study coil was successfully implanted at the target site with 30% angiosuite packing density.Immediate post-procedure modified raymond-roy classification score was class ii: residual neck = persistence of any portion of the original defect of the arterial wall but without opacification of the aneurysmal sac. There were no reported study device deficiencies.180-day follow-up visit was performed in the clinic on (b)(6) 2021; his mrs score was 0.The complaint coil remains implanted and is thus not available for evaluation.

Additional Manufacturer Narrative
Manufacturer¿s ref.No: (b)(4).The purpose of this mdr submission is to include the additional event information received on 11-jan-2022.[additional information]: the event was reported via the sterling study, a 51-year-old male patient with a history of hypertension, coronary artery disease (cad), and active smoking underwent coil embolization of an unruptured anterior cerebral artery (aca) on (b)(6) 2021 and experienced a thromboembolic event during the procedure.Medication was administered as treatment. On 11-jan-2022, additional information was received.Per the information, the physician attributed the cause of the event to a complex anatomy-fenestrated aca a1 segment. The relationship of the study coil to the reported event was deemed possible. When the thrombus was identified, immediate anti-thrombotic medication was administered until the clot was resolved.Intra-arterial integrilin bolus (2mg/ml with 6.8ml) and perfusor 0.75mg/ml 12ml/h for 6 hours were administered per standard protocol. The thrombus was not flow-limiting nor did it embolize. There was no coil protrusion into the parent vessel that may have contributed to the event. There was no evidence of parent artery nor side branch vessel occlusion. The thrombus did not result in an infarction. The event did not require prolonged hospitalization. The last two coils were associated with the microcatheter kickback; per the case report form (crf), a 2mm x 4cm target nano coil (stryker) (competitor) and the complaint coil were the last two coils placed.Based on the additional information received on 11-jan-2022, positioning problem was added in the medical device problem code to capture the reported microcatheter kickback associated with the cerenovus coil. The file will be re-reviewed if additional information is received at a later date. The manufacturer will submit a supplemental report if new facts arise which materially alter information submitted in a previous mdr report.Additional information will be submitted within 30 days of receipt.
