Intuitive surgical, inc.(isi) has not received the reducer for evaluation. Therefore, the root cause of the customer reported failure mode has not been determined.A follow-up mdr will be submitted if the product is returned and evaluated and/ or if additional information is received.A review of the site's complaint history does not show any additional complaints related to this product and/or this event. In addition, this is an accessory, which does not appear in the logs. Therefore, an instrument log review of the product related to the complaint cannot be performed. No image or video clip for the reported event was submitted to isi for review. This event is being reported because the metal part of the reducer¿s tip was separated and could fall into the patient¿s body.At this time, it is unknown what caused the tip to separate from the reducer. While there was no harm or injury to the patient, the reported failure mode could likely cause or contribute to an adverse event if it were to recur as unintended items falling into the patient may require surgical intervention.

|
It was reported that during a da vinci-assisted ventral hernia surgical procedure, the customer used the reducer on a 12mm trocar; however, the metal part at the tip of reducer was separated.Although the metal part did not fall inside the patient's body, the customer was concerned that it could.A backup reducer was used to continue the procedure. The procedure was completed with no report of patient injury.Intuitive surgical, inc.(isi) followed up with the initial reporter and obtained the following additional information: the reducer was inspected prior to use with no damage observed. The customer confirmed that nothing fell inside of the patient and that there was no patient injury/harm.
