Event Description
It was reported that the sterile inner package of the pinnacle cup was stuck against the outer box and was torn when opening, thus being unsterile.

Event Description
Additional information received indicated that the deficiency was discovered upon opening the package of the pinnacle cup by the assisting nurse.
