Additional Manufacturer Narrative
H10: manufacturing review: the device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: the physical device was not returned for evaluation. Two images and one video were reviewed. The first image shows a venogram of the left thigh below the femoral head. This demonstrates segmental occlusive disease of the left femoral vein. The second image shows an angioplasty balloon being inflated near the left femoral head.A wire is seen coming up and over from the right groin. The balloon does not appear completely inflated in this image.A small area of the balloon appears folded or twisted.A video shows an angioplasty balloon being inflated at a similar location as the second image. The balloon appears to inflate abnormally, almost as if the balloon is twisted or folded onto itself. Therefore, based on the images and video review, the reported longitudinal balloon rupture and difficult to remove are kept inconclusive. The investigation is confirmed for the identified material deformation since the review mentions the balloon does not appear completely inflated and small area of the balloon appears folded or twisted. One photo was provided and reviewed. The photo shows the ultraverse 035 balloon which is deflated and has blood residue. No evidence of balloon rupture can be seen. Therefore, based on the photo review, the reported longitudinal balloon rupture and difficult to remove are kept inconclusive. Therefore, the investigation is inconclusive for the reported longitudinal balloon rupture and difficult to remove since no evidence for the longitudinal balloon rupture and difficult to remove were provided. The investigation is confirmed for the identified material deformation since the images and video review mentions the balloon does not appear completely inflated and small area of the balloon appears folded or twisted.A definitive root cause for the reported longitudinal balloon rupture and difficult to remove and the identified material deformation could not be determined based upon the provided information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.H11:section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : device not returned.
