Additional Manufacturer Narrative
Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report. If the information is unknown, not available or does not apply, the section/field of the form is left blank.Review of the device history record(s) showed that there were no issues during the manufacture of this product, and any sub-components, which would contribute to this complaint condition.Part # 07.704.003s, synthes lot # ds7008141, supplier lot # ds7008141, release to warehouse date: 30 sep 2021, expiration date: 28 jul 2023, supplier: (b)(4).No ncr's were generated during production. The product was returned to depuy synthes for evaluation. The depuy synthes team forwarded the device to dsm where a visual inspection of the returned device was conducted.Dsm reviewed all related device history records and risk documentation associated with the complaint condition which reported that bone void filler did not dispense from the pouch after mixing. The returned pouch was observed to have a cracked luer lock on the rotary pouch where the solution syringe is attached.Hardened material in pouch indicated that solution was injected and a reaction occurred. However, due to the failure to dispense identified in the complaint description it is assumed that an inadequate amount of solution was injected as a result of the cracked luer. There were no discrepancies related to this failure mode during the manufacturing of the lot from which the product was received. The crack in the luer was visible during an inspection of the pouch and consisted of a crack along the length of the luer lock.A dimensional inspection was unable to be performed due to not being applicable to the complaint condition. The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended forces. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications. The overall complaint was confirmed as the observed condition of the norian drillable inject 3cc-sterile, p/n: 07.704.003s would contribute to the complained device issue. There is no indication that a design or manufacturing issue has caused the complaint condition and hence the root cause cannot be determined.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities. If information is obtained that was not available for this medwatch, a follow-up medwatch will be filed as appropriate.
