Future date of explantation: february 8, 2022.Since the device has not been returned for analysis, no product failure analysis can be conducted, and no determination of possible contributing factors can be made.As such, the investigation will be closed. If the complaint device is received in the future, the investigation will be reopened and conducted as appropriate.A manufacturing record evaluation (mre) was performed, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures.Reason for device explant and/or reoperation: material rupture, lymphadenopathy.Manufacturer¿s reference number: (b)(4).

Event or Problem Description
It was reported that a (b)(6) caucasian female patient, underwent primary breast augmentation with two 350cc mentor memoryshape breast implants.Post-operatively, via mammogram, the patient was diagnosed with right breast implant rupture and silicone in her lymph nodes and armpit.Prior to the mammogram, the patient initially noticed a lump in her right breast and swollen lymph nodes. As a result, the patient has been scheduled for implant removal on (b)(6).

Additional Manufacturer Narrative
On february 18, 2022, the mentor failure analysis lab received the device for evaluation. On march 4, 2022, the product investigation was completed.Device investigation summary: the product was returned to mentor for evaluation.Mentor then conducted a visual inspection and microscopic examination of the returned device.Visual analysis of the returned sample determined that the mentor memoryshape¿ tm+ 350cc breast implant was found to be ruptured and received in two parts.Microscopic examination was performed on the edges of the rupture, and parallel striations were found in an area of the tear on the anterior view, measuring approximately 0.1 cm.Parallel striations are consistent with markings made by a sharp object perforating the implant shell. The cause of the rupture in the remaining area of the tear could not be identified.A manufacturing record evaluation was performed for the finished device lot number, and no non-conformances were identified.Lymphadenopathy or palpable lymph nodes may be the result of bacterial or viral infection, neoplasm (primary or metastatic), or as the result of an idiopathic inflammatory process or foreign body reaction. There have been reports regarding movements of silicone gel material to lymph nodes even with or without evidence of rupture leading to lymphadenopathy.A number of epidemiology studies have evaluated large populations of women with breast implants from a variety of manufacturers and implant models. The studies do not taken together, support an association of breast implants and a diagnosed rheumatic disease.Other than one study, these studies do not distinguish whether the women had ruptured or intact implants.Because there were no supporting pathology reports or physician consultation summaries forwarded to product evaluation, we are unable to confirm the presence of or the possible etiology of the reported lymphadenopathy.Lymphadenopathy is a known complication associated with this surgery and is referenced in our current product insert data sheet.A second product was received (lot-7281789).No adverse events were reported for this concomitant (contralateral) device, therefore no further analysis is required. As part of mentor¿s quality process, all devices are manufactured, inspected, and released to approved specifications.

Additional Manufacturer Narrative
On april 4, 2022, mentor became aware that the patient was also diagnosed with left breast implant rupture during the revision surgery on (b)(6), 2022.The patient underwent total capsulectomy and bilateral replacement with the following devices: (right) 545cc mentor memorygel breast implant catalog: smpx545 lot: 9555950 sn: (b)(6) and (left) 545cc mentor memorygel breast implant catalog: smpx545 lot: 9555950 sn: (b)(6).Rupture on the left breast implant will be reported under mrn: 1645337-2022-04076.This medwatch form is for the right breast prosthesis.
