Event or Problem Description
One resolute onyx coronary drug eluting stent was attempted to be used to treat a moderately tortuous, moderately calcified lesion in the mid right coronary artery (rca). The device was inspected prior to use with no issues noted.Negative prep was performed with no issues noted. The device did not pass through a previously deployed stent.Excessive force was not used during delivery.A guidewas engaged in the artery with a guide extension catheter for extra support through the vessel with a non-medtronic guide wire.It was reported that when the stent was travelling on the guide wire it got stuck on the ramp of the guide extension catheter and dislodged from the balloon. It was stated that all equipment was taken out of the patient body with no issues. No patient injury reported.
