Event Description
During the procedure, a transseptal puncture was performed using a non-abbott transseptal guidewire through a brk needle.Both the devices were then removed from the patient and when removing the non-abbott guidewire from the brk needle, a small piece of metal came out of the proximal end of the brk needle.A further look revealed it was a non-beveled metal piece that had a lumen.No pieces were thought to have detached in the patient, however, it could not fully be confirmed.The procedure was continued with a different brk needle and there were no adverse consequences to the patient.The metal piece was not thought to have been from the non-abbott transseptal guidewire.The brk needle was initially pre-shaped and no stylet was used.
 
Manufacturer Narrative
One brk transseptal needle and stylet were received for evaluation.The packaging tray was also returned.A metal tube with rounded edges and an inner lumen was returned with the brk needle/stylet assembly.Additional analysis revealed the returned metal tube was identified by the supplier as an outer cannula of the stylet.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.
