Event Description
It was reported that deflation failure occurred.A 4.00 x 12 synergy ii drug-eluting stent was used in a procedure for treatment.However, after the stent was deployed, the stent balloon would not fully deflate even after several attempts.The balloon would not re-enter the guide catheter, so the devices were pulled out all together.The procedure was completed using the original device.There were no patient complications nor injuries reported.
