Event Description
The user facility reported that when preparing the device for use, the front panel light emitting diode (led) of the visera pro xenon light source would blink without lighting the lamp at startup.There was no patient or user harm due to the event.
 
Manufacturer Narrative
The subject device was sent to an olympus service center for evaluation.During device inspection and testing, the customer's complaint could not be reproduced.However, service found that high brightness switching did not occur due to sticking of the mesh turret.The investigation is ongoing; therefore, a root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The dhr was unable to be reviewed for this device since the device is over 15 years old.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, it was not possible to determine whether the phenomenon indicated occurred due to the failure of the device because the occurrence of the phenomenon cannot be confirmed.A definitive root cause cannot be identified.Olympus will continue to monitor the field performance of this device.
