Event Description
|
Medtronic received a report that a patient died 11 days after pipeline implant due to delayed aneurysm rupture in the c4 segment of the internal carotid artery.The procedure had been completed without any problems and there were device malfunctions. the devices were prepared according to the instructions for use (ifu).The patient was undergoing treatment for an unruptured, saccular aneurysm located in the c4 segment of the left internal carotid artery.The max diameter was 12mm, and the neck diameter was 7mm.The patient's vessel tortuosity was moderate.The landing zone was 4.5mm distal and 4.9mm proximal. dual antiplatelet treatment had been administered.

Additional information received reported that the cause of the rupture was unknown.The procedure was observed on the web, the adhesion of the device appeared good.Concomitant coils were not used.An autopsy was not performed.The causal relationship between the patient's death and the device was not clear.Before the patient passed away, the steroid had been reduced due to the patient's condition becoming better several days ago.The patient had been hospitalized for postoperative medical treatment when it occurred.

Additional information received confirmed that the location of aneurysm was at cavernous (c4) segment of ica.The pipeline did not extend into the c5.It was c2, from 3 to 4.Dapt was aspirin and clopidogrel.
